Don't forget to enclose a check for $20.00.  Note, if you’re competing in more than one class you will be required to fill out an additional form, and fee for each additional class is $10.00.  Also, it will save you time at sign-up on race weekend if you'd print a copy of the entry form and fill it out before you come to the sign up table.  The forms are listed at the below link.
